Description
Join the conversation to learn about NYS-MEP student forms and how they’ve changed with the new 2022-25 Service Delivery Plan (SDP). We will focus on:
- Academic Service Intensity Rubric (ASIR)
- Service Level Requirements Chart
- School Year Student Intake Form (SIF)
- Out of School Youth (OSY) Profile
- Supplemental Service Codes
- Personal Learning Plan (PLP)
Leave today with an understanding of the forms that you will interact with throughout the year.
